ONLINE PATIENT REGISTRATION ?

IOH Medical Centre is now pleased to offer our patients convenient online pre-registration. This service will allow us to validate all demographic information prior to your visit. On the day that you arrive, we will simply copy your insurance card and have you sign a consent form.

Online Pre-Registration is available for appointments 24 or more hours from now. This is to ensure there is adequate time to process the registration and avoid patient delays or inconvenience on the day of your visit.
